AHN Account Contact Change Form
>This form is to submit the currently authorized list of persons who should be listed as accounts contacts in our system.

>They will be authorized to call into our support team and request changes to the account information and the website.

> They will be able to access AtHomeNet billing information and will be able to add and remove other authorized contacts going forward.

> We recommend having a maximum of 3 account contacts per website.

> Due to security precautions, administrative logins will not be created for the AtHomeNet Website.  Existing Administrators can create a Login for these individuals by adding a new profile in the Address Book that has a Security Level of 1.

> When this form has been confirmed through our support department, you will need to reach out to any pending cases you may have previously submitted, if you wish to prevent these cases from being closed.

** Each community website has its own account, and management companies have their own website account as well. Please be specific about whom should be listed as a contact for which community website. **
